HACAP understands the importance of meeting health and nutritional needs during critical times of developmental growth. We provide a variety of services for maternal, infant and child health. Together with local medical and human service organizational partners, low-wage families and individuals can access our programs by visiting one of our Linn County Service Centers.
Program Profiles
Care for Kids Child Health program facilitates achievement and maintenance of optimal health in children under the age of twenty-one receiving Medicaid or Title IX. Care for Kids promotes families as independent health consumers, orientating them to the local fee-for-service medical community. Care coordination for children in Linn County is provided by informing families about resources available to them regarding their health care needs and supporting families in making informed health care decisions.
Maternal Health provides prenatal education, nutritional education and social assessment for pregnant women. Home visits are offered to women with high-risk pregnancies.
hawk-i is a low-cost or no-cost health insurance program. Benefits include doctor’s visits, dental care, eyeglasses, vaccinations, well-child visits, inpatient hospital services, prescription drugs, outpatient services, mental health services and case management support.
Well Child Screening Clinic provides well child care for children from birth through 21 years of age. Comprehensive services include developmental screening, hearing and vision testing, nutrition assessment, social assessment, immunizations and lead testing. In addition, oral health care is provided including dental screening and fluoride varnish applications.
WIC is the USDA funded Supplemental Nutrition Program for Women, Infants, and Children. It provides nutrition education and healthy food packages to help families meet their nutritional needs.
